They are offered by private insurance companies in some states, but not all. They include all the benefits of Original Medicare Part A, Part B, and Part D, and may have additional benefits depending on the state. Most Dual Special Needs Plans are either HMOs or PPOs, which have different rules for choosing doctors and specialists.
